Completing the PLF form
The website says to submit only ONE form for family/ household. Husband got his Moderna vaccine in Jan / Feb. and spouse got the Pfizer in April. Do I need to complete another PLF form?

RTBones MemberI think we need a little more info here – assuming the PLF you refer to is a Passenger Locator Form. For which country? Just based on what you’ve written – if you are entering a country with a PLF requirement, it is generally one PLF/family per trip you take.
