FlyingBlue Newbie Help

I have never redeemed miles on FlyingBlue so I had a few questions. To give you context – I generally earn miles on my Chase Sapphire and redeem with United, or from my delta amex cards for redeeming on delta directly.
1) I just opened up my FB account a few weeks ago. I read that there were some issues in the past year in which [B]new[/B] accounts were flagged or closed if the new account holder just transferred MR miles in, and the user either had the flight canceled or had to go to the airport to book the tickets. Tiffany said on some other posts that it hasn’t been a problem recently, but want to confirm that is still the case for newly opened accounts. If this is an issue for new accounts still, would you advise calling the European call centers to book the tickets and transfer miles while on the phone with the agent? I’m worried my miles will be stuck in limbo in my FB account!
2) I have miles I would want to transfer to FB from Chase Sapphire, my platinum amex, and my SPG amex. This may be a dumb question – but can I transfer points from all three directly into my FB account? Sort of like how you can transfer from Chase Sapphire directly into United?
3) Are there issues with phantom award space? I am trying to book a flight from NYC to Biarritz for October and I saw some availability in business class for 62,500 miles. I was worried those seats may not be available and my miles will be ‘stuck’ in FB, based on some posters comments that FB never has premium space available online.
