I enjoy the instant upgrade feature because its a easy way to use my Ritz Carlton Credit Card Travel Credit- just booked 2 - one way rewards flights from PHL to MCO in March and I was offered instant frist class upgrade for 171 each - took the upgrade and used the credit to cover the cost.
My wife and I recently flew with AA from MAD to MIA in premium economy. I tracked the upgrade cost for business for about 6 weeks leading up to the flight. It started at about $2500, dropped to $1700 two weeks before the flight with multiple open seats, but then rose to $3600 as seats filled.
I've purchased the instant upgrades on two flights DFW-HNL in the last six months. Since this is a popular route it wasn't the greatest deal but it was definitely cheaper than buying business class at the time of booking. However, one annoyance is that in both cases, the miles/LPs did not post for the upgrade portion so I had to make a request online for the miles which they agreed were owed and posted them.
I purchase the cash upgrades when flying domestic, as long as I think they are value for money. I don't recall paying more than $340 per segment to fly from DC to any point in the USA the last two years. This year, I think AA's algorithm has gone crazy. For my annual business trips in March where I connect in ORD and fly from ORD to LAX and ORD to LAX, the upgrade offers are priced at ~$1,380 per segment. I can't fathom anyone in their right mind paying this for what is tantamount to a premium economy service. So I'm holding to sanity will win out and price points will return to the <=$300 levels.
It's has become the only decent way of getting upgraded. Generally using miles isn't a good deal. A few caveats. The offers are usually downright outrageous months out-like $700 for RDU/CLT. Closer to departure date (within 30 days or less) the offers become more reasonable. Second, I am finding that if you hit "No Thanks" on the mobile app (because what sane person would spend $700 on a one hour flight) the offer will disappear from the mobile app but will remain in the desktop app. Third, transferring them to another flight during irregular operations is mixed. Sometimes they're treated as paid first and other times you go back onto the upgrade list but will be refunded if you don't clear. You have to find an agent that can reaccommodate you into an open premium seat.
I have been an EP for the last 8 years and have seen complementary upgrades become virtually nonexistent (unless you are a Key). I have flown weekly from DFW to DCA and LGA for the last 5 years and as a EP, I am never upgraded anymore (twice in the last 24 months).
Trying to use a systemwide to travel on a long haul flight is all but impossible. So why be loyal to AA?
Well I made the decision to no longer be loyal and give up my AA EP status at the end of this period.
AA treats its elites like trash and I am relieved to not be on the hamster wheel chasing worthless EP status anymore.
Interesting - I have a 100% success rate where I’ve upgraded using cash on one segment of a two segment itinerary and when rebooking myself I am rebooked into first on both segments. I guess YMMV, and depends on whether seats are available but it’s so far always worked in my favor.
I don’t often take advantage of the cash offers but as a lowly Gold I only get one free checked bag. Sometimes a MCI-DFW or LAX-PHX upgrade is in the ~$70 range and if I will have a second bag it’s a no-brainer since I’d spend $50 on the second bag.
Plus, then the checked bags get priority tags, which… occasionally means something.
I take advantage of this for business travel when I'm flying international longhaul but need to be conscious of a project budget (if I couldn't afford a premium cabin ticket outright.) I'll upgrade the long flight only, and often only the outbound. I wish you could specify which LEG you want a premium cabin on at time of booking, not just the entire married segment. I'll gladly roll the dice on an upgrade from ORD to CLT, but I'd like to confirm a premium cabin for CLT to FRA (for example.)

Same. When I do same day confirmed, it puts me in F. At least 3x so far.
I was familiar with having to manually request refunds and then trip credits for paid upgrades on flights I cancel. But last week I canceled an itinerary for which I had purchased an instant upgrade. The process informed me that the value of the instant upgrade would be applied to my account as a trip credit within 7 days. It posted to my account the next day and is valid for a year.
I have no status, am retired, fly AA maybe three trips per year, and actually like this system. I upgrade maybe half the time; certainly any time the upgrade costs $1.00 per flying minute or less. Just last week, I flew BDL-CLT-SFO, and the CLT-SFO six hour leg only cost approx $270 additional. That’s a no brainer.
This has absolutely been my experience as well. I’m also an EP and have been unable to score complimentary upgrades on domestic flights or have systemwide upgrades clear, despite trying to use them more than a dozen times in the past year. These “perks” are entirely an illusion, and there is no longer a reason to be loyal to the airline. Additionally, the food on international itineraries has noticeably suffered in recent years. The US carriers have become an embarrassment.
